DORA Register of Information: [Financial entity or group name]
Working file and design record for the register of information on ICT
third-party contractual arrangements that Article 28(3) of the Digital
Operational Resilience Act (Regulation (EU) 2022/2554, DORA) requires,
built on the standard templates of Commission Implementing Regulation
(EU) 2024/2956 (templates B_01.01 to B_07.01 and B_99.01). It covers
the entities in scope, contractual arrangements, providers and
identifiers, ICT service supply chains and ranking, functions and
their criticality, the assessment of services that support critical
or important functions, concentration and exit, the Article 30
contract check, data quality controls and the yearly submission. DORA
applies from 17 January 2025; check the consolidated texts on EUR-Lex
and your competent authority's reporting instructions. Replace every
[bracketed] value and every row marked Example. This is a
documentation format, not legal advice.

Contents
1. Purpose, scope and legal basis
1.1 Purpose
The register records every contractual arrangement under which
[Entity] uses ICT services from an ICT third-party service provider. It
serves three users: the entity's own ICT third-party risk management,
the competent authority's supervision, and the European Supervisory
Authorities (ESAs) for oversight of critical ICT third-party service
providers. The register distinguishes arrangements that cover ICT
services supporting critical or important functions from those that do
not.
1.2 Legal basis
| Provision | What it requires (paraphrase) | Where handled |
|---|
| DORA Article 28(1) | Manage ICT third-party risk inside the ICT risk management framework; the entity stays fully responsible | Section 2 |
| DORA Article 28(2) | Strategy on ICT third-party risk with a policy on services supporting critical or important functions (not for microenterprises and simplified-framework entities) | Section 2 |
| DORA Article 28(3) | Maintain the register at entity, sub-consolidated and consolidated level; report yearly on new arrangements, provider categories, arrangement types, services and functions; provide the register on request; inform the authority in time of planned critical or important arrangements and of functions that become critical or important | Sections 4 to 9, 14 |
| DORA Article 28(4) to (8) | Pre-contract assessment, due diligence, information security standards, audit planning, termination rights and exit strategies | Sections 9, 10 |
| DORA Article 29 | Preliminary assessment of ICT concentration risk, including subcontracting chains and third-country providers | Section 10 |
| DORA Article 30 | Minimum contractual provisions, with additional provisions for critical or important functions | Section 11 |
| Implementing Regulation (EU) 2024/2956 | Standard templates, ranking of providers in the supply chain, data quality principles, identifiers (LEI or EUID), scope at group level | Sections 3 to 13 |
| Delegated Regulation (EU) 2024/1773 | Content of the policy on contractual arrangements for critical or important functions | Section 11 |
| Delegated Regulation (EU) 2025/532 | Elements to determine and assess when subcontracting services that support critical or important functions | Section 7 |
Implementing Regulation (EU) 2024/2956 was adopted on 29 November 2024,
published in the Official Journal on 2 December 2024 and entered into
force on the twentieth day after publication. DORA has applied since 17
January 2025 without a transitional period. Check the consolidated texts
for amendments.

The EBA data point model and reporting taxonomy use the same template
codes. Check the current reporting framework release on the EBA website
before each submission.
4. Entities in scope
4.1 Entity maintaining the register (B_01.01)
| LEI | Name | Country | Type of entity | Competent authority | Date of reporting |
|---|
| [LEI] (Example) | Example Bank AG | DE | Credit institution | [Authority] | YYYY-MM-DD |
4.2 Entities in scope of consolidation (B_01.02)
| LEI | Name | Country | Type of entity | Hierarchy in group | Parent LEI | Total assets (Annex IV) | Currency |
|---|
| [LEI-1] (Example) | Example Bank AG | DE | Credit institution | Ultimate parent | Not applicable | [Value] | EUR |
| [LEI-2] (Example) | Example Invest GmbH | DE | Investment firm | Subsidiary | [LEI-1] | [Value] | EUR |
| [LEI-3] (Example) | Example IT Services GmbH | DE | ICT intra-group service provider | Subsidiary | [LEI-1] | Not applicable | EUR |
4.3 Branches (B_01.03)
| Branch code | Head office LEI | Branch name | Country |
|---|
| BR-AT-01 (Example) | [LEI-1] | Example Bank AG, Vienna branch | AT |
5. Contractual arrangements
5.1 General information (B_02.01)
| Reference number | Type of arrangement | Overarching reference | Annual expense or estimate | Currency |
|---|
| CA-0001 (Example) | Standalone | Not applicable | 2,400,000 | EUR |
| CA-0002 (Example) | Standalone | Not applicable | 1,850,000 | EUR |
| CA-0003 (Example) | Standalone (intra-group provider) | Not applicable | 900,000 | EUR |
| CA-0004 (Example) | Overarching framework | Not applicable | 300,000 | EUR |
| CA-0005 (Example) | Subsequent or associated | CA-0004 | 120,000 | EUR |
5.2 Specific information (B_02.02)
| Reference | User entity | Provider | Function | Service type (Annex III) | Start | End | Notice (entity / provider) | Governing law | Provision country | Data stored | Data location (rest / processing) | Data sensitiveness | Reliance |
|---|
| CA-0001 (Example) | [LEI-1] | PRV-001 | F1 | S19 SaaS | YYYY-MM-DD | YYYY-MM-DD | 180 / 365 days | DE | DE | Yes | DE / DE, IE | High | Full reliance |
| CA-0002 (Example) | [LEI-3] | PRV-002 | F1, F2 | S17 IaaS | YYYY-MM-DD | Open-ended | 90 / 365 days | IE | IE | Yes | IE, NL / IE, NL | High | Material reliance |
| CA-0003 (Example) | [LEI-1] | PRV-003 (intra-group) | F2 | S14 ICT operation management | YYYY-MM-DD | Open-ended | 90 / 90 days | DE | DE | No | Not applicable | Not applicable | Full reliance |
| CA-0005 (Example) | [LEI-1] | PRV-004 | F3 | S04 ICT security management | YYYY-MM-DD | YYYY-MM-DD | 90 / 180 days | DE | DE, PL | Yes | DE / PL | Medium | Material reliance |
Data locations and provision countries are shown combined for
readability; in the live register add one row per country of storage,
processing or provision. Report only the service type code (for example
S19); labels are shown here for readers. One row exists for each
combination of entity, function and service type. CA-0002 therefore needs two rows (F1 and F2) in the live register.
5.3 Intra-group links (B_02.03)
| Intra-group arrangement | Linked external arrangement |
|---|
| CA-0003 (Example) | CA-0002 |
The intra-group IT company (LEI-3) signs the cloud contract CA-0002 and
provides operations to the bank under CA-0003, so both arrangements
belong to the same chain.
5.4 Signing and using entities (B_03.01, B_03.02, B_03.03, B_04.01)
| Reference | Signing entity (B_03.01) | Signing provider (B_03.02) | Group entity providing services (B_03.03) | Entities using the services (B_04.01) |
|---|
| CA-0001 (Example) | [LEI-1] | PRV-001 | Not applicable | [LEI-1], [LEI-2] |
| CA-0002 (Example) | [LEI-3] | PRV-002 | Not applicable | [LEI-3], [LEI-1], [LEI-2] |
| CA-0003 (Example) | [LEI-1] | PRV-003 | [LEI-3] | [LEI-1], BR-AT-01 |
6. ICT third-party service providers
| Provider ID (internal) | Identification code and type | Additional code and type | Legal name | Name in Latin alphabet | Type of person | HQ country | Total annual expense | Ultimate parent code and type |
|---|
| PRV-001 (Example) | [LEI] (LEI) | [EUID] (EUID) | Example Core Banking SaaS SE | Same | Legal person | DE | 2,400,000 EUR | [LEI] (LEI) |
| PRV-002 (Example) | [LEI] (LEI) | Not available | Example Cloud EMEA Ltd | Same | Legal person | IE | 1,850,000 EUR | [LEI] (LEI) |
| PRV-003 (Example) | [LEI-3] (LEI) | [EUID] (EUID) | Example IT Services GmbH | Same | Legal person | DE | 900,000 EUR | [LEI-1] (LEI) |
| PRV-004 (Example) | [LEI] (LEI) | Not available | Example MSSP Sp. z o.o. | Same | Legal person | PL | 420,000 EUR | [LEI] (LEI) |
| PRV-005 (Example) | [LEI] (LEI) | Not available | Example Data Centre BV | Same | Legal person | NL | Not applicable (subcontractor) | [LEI] (LEI) |
Total annual expense for PRV-004 sums CA-0004 and CA-0005 (300,000 plus
120,000). Check LEI status against the GLEIF database at each update; a
lapsed LEI fails validation.
7. ICT service supply chains and ranking
7.1 Chain diagram
The diagram shows the worked example chain for the core banking service
(CA-0001) and the cloud service (CA-0002).
7.2 Supply chain register (B_05.02)
| Reference | Service type | Provider | Rank | Recipient of subcontracted service |
|---|
| CA-0001 (Example) | S19 SaaS | PRV-001 | 1 | Not applicable (direct) |
| CA-0001 (Example) | S19 SaaS | PRV-002 | 2 | PRV-001 |
| CA-0001 (Example) | S19 SaaS | PRV-005 | 3 | PRV-002 |
| CA-0002 (Example) | S17 IaaS | PRV-002 | 1 | Not applicable (direct) |
| CA-0002 (Example) | S17 IaaS | PRV-005 | 2 | PRV-002 |
7.3 Subcontracting assessment
For each subcontractor that supports a critical or important function,
record the due diligence and conditions that Delegated Regulation (EU)
2025/532 asks the entity to determine: whether subcontracting is
permitted and on what conditions, how the direct provider monitors the
subcontractor, the entity's audit and information rights along the
chain, the notice for material changes and the right to object or
terminate.
| Subcontractor | Function supported | Subcontracting permitted in contract | Monitoring by direct provider | Entity audit rights in chain | Material change notice | Termination right if objected change proceeds |
|---|
| PRV-002 under CA-0001 (Example) | F1 | Yes, listed in annex 4 | Quarterly report and annual certification | Pooled audit with provider | 90 days | Yes |
| PRV-005 under CA-0001 (Example) | F1 | Yes, as data centre of PRV-002 | Through PRV-002 attestations | Through PRV-002 | 90 days | Yes |
8. Functions and criticality
8.1 Criticality criteria
| Criterion | Question | Evidence |
|---|
| Financial performance | Would disruption materially impair financial results? | Business impact analysis |
| Soundness and continuity | Would disruption materially impair the continuity of services to clients? | BIA, recovery objectives |
| Regulatory compliance | Would failure materially impair compliance with authorisation conditions or financial services law? | Compliance assessment |
| Group dependency | Do other group entities rely on the function? | Group service catalogue |
8.2 Function register (B_06.01)
| Function ID | Entity LEI | Licensed activity (Annex II) | Function name | Critical or important | Reasons | Last assessment | RTO (hours) | RPO (hours) | Impact of discontinuing |
|---|
| F1 (Example) | [LEI-1] | Taking deposits and other repayable funds | Account management and payments | Yes | Client continuity; regulatory compliance | YYYY-MM-DD | 4 | 1 | High |
| F2 (Example) | [LEI-1] | Lending | Loan origination | Yes | Financial performance | YYYY-MM-DD | 24 | 4 | Medium |
| F3 (Example) | [LEI-1] | Taking deposits and other repayable funds | Security monitoring | Yes | Supports F1 and F2 security | YYYY-MM-DD | 8 | 1 | High |
| F4 (Example) | [LEI-2] | Portfolio management | Client reporting | No | Can run manually for two weeks | YYYY-MM-DD | 72 | 24 | Low |
RTO and RPO are reported in whole hours: report 1 where the objective is
below one hour and 0 where it is not defined. F1 has an internal RPO of
15 minutes, which is reported as 1; keep the precise value in the
business impact analysis. The same function performed under two
licensed activities needs two identifiers. Align recovery objectives with the Business Continuity
(compliance/business-continuity) plan and the Disaster Recovery Plan
(ops/disaster-recovery-plan).
9. Assessment of ICT services supporting critical or important functions
| Reference | Provider | Service type | Substitutability | Reason if not substitutable | Last audit | Exit plan | Reintegration possible | Impact of discontinuing | Alternatives identified | Alternative provider |
|---|
| CA-0001 (Example) | PRV-001 | S19 | Highly complex substitutability | 3 (both lack of real alternatives and migration difficulty); internal note: proprietary data model, 18-month migration | YYYY-MM-DD | Yes | Highly complex | High | Yes | [Provider B] |
| CA-0002 (Example) | PRV-002 | S17 | Medium complexity | Not applicable | YYYY-MM-DD | Yes | Difficult | High | Yes | [Provider C] |
| CA-0003 (Example) | PRV-003 | S14 | Medium complexity | Not applicable | YYYY-MM-DD | Yes | Easy | Medium | Yes | [Provider E] |
| CA-0005 (Example) | PRV-004 | S04 | Easily substitutable | Not applicable | YYYY-MM-DD | Yes | Easy | Medium | Yes | [Provider D] |
The reason column is a closed list of three values (1 lack of real
alternatives, 2 difficulties in migration or reintegration, 3 both) and
is mandatory when the service is marked not substitutable or highly
complex to substitute. Keep the narrative in the assessment record, not
in the register. Every arrangement that supports a critical or
important function needs a row here, including intra-group ones such as
CA-0003.
9.1 Assessment checklist before contracting (Article 28(4))
- Confirm whether the service supports a critical or important function
and record the function identifier.
- Confirm that supervisory conditions for contracting are met.
- Identify and assess the risks, including whether the arrangement adds
to ICT concentration risk (section 10).
- Complete due diligence on the provider and record the outcome in the
Supplier Security Assessment (
compliance/supplier-security-assessment).
- Identify and assess conflicts of interest.
- Confirm the provider applies appropriate information security
standards, and for critical or important functions the most current
and highest-quality standards (Article 28(5)).
- Set the audit frequency and areas on a risk basis (Article 28(6)).
10. Concentration risk and exit
10.1 Concentration view
| Provider or group | Arrangements | Functions supported | Share of ICT spend | Substitutability | Third-country exposure | Concentration rating |
|---|
| PRV-002 and parent (Example) | CA-0002 direct; CA-0001 at rank 2 | F1, F2 | 3,000,000 of 5,570,000 EUR (54%) | Medium | None (EU regions) | High |
| PRV-001 (Example) | CA-0001 | F1 | 2,400,000 of 5,570,000 EUR (43%) | Highly complex | None | High |
Total spend in the example is the sum of CA-0001 to CA-0005 (2,400,000
plus 1,850,000 plus 900,000 plus 300,000 plus 120,000, which is
5,570,000 EUR). The PRV-002 figure counts direct spend of 1,850,000 and
an estimated 1,150,000 of PRV-001 charges attributable to hosting; mark
estimates as such in the live register. Shares overlap because the same
spend is seen at two ranks.
10.2 Exit plan register
| Reference | Exit trigger (Article 28(7) grounds and 28(8) exit strategy) | Target state | Transition period in contract | Data return format and timing | Last test | Test result | Owner |
|---|
| CA-0001 (Example) | Provider failure, material breach, supervisory obstacle | Migrate to alternative SaaS | 24 months | Full export in documented schema within 30 days | YYYY-MM-DD (tabletop) | Gaps in archive data mapping | [@core-banking-owner] |
| CA-0002 (Example) | Provider failure, significant breach, evidenced security weakness; strategic exit (region withdrawal) under the 28(8) strategy | Second provider using same IaC | 12 months | Object storage replication | YYYY-MM-DD (partial failover) | Pass | [@platform-lead] |
11. Key contractual provisions check
| Element (paraphrase) | Article | Applies to | CA-0001 (Example) | CA-0002 (Example) | CA-0005 (Example) |
|---|
| Complete description of functions and services, and whether and how subcontracting is allowed | 30(2)(a) | All | Yes | Yes | Yes |
| Locations of service provision and data processing and storage, with advance notice of changes | 30(2)(b) | All | Yes | Yes | Gap |
| Data protection provisions on availability, authenticity, integrity and confidentiality | 30(2)(c) | All | Yes | Yes | Yes |
| Access, recovery and return of data on insolvency, resolution, discontinuation or termination | 30(2)(d) | All | Yes | Yes | Yes |
| Service level descriptions and their updates | 30(2)(e) | All | Yes | Yes | Yes |
| Incident assistance at no cost or at a cost fixed in advance | 30(2)(f) | All | Yes | Gap | Yes |
| Full cooperation with competent and resolution authorities | 30(2)(g) | All | Yes | Yes | Yes |
| Termination rights and minimum notice periods | 30(2)(h) | All | Yes | Yes | Yes |
| Participation in security awareness and resilience training | 30(2)(i) | All | Gap | Gap | Yes |
| Full service levels with precise quantitative and qualitative targets | 30(3)(a) | Critical or important | Yes | Yes | Yes |
| Notice and reporting duties on developments that could affect service | 30(3)(b) | Critical or important | Yes | Yes | Yes |
| Provider business contingency plans and security measures, tested | 30(3)(c) | Critical or important | Yes | Yes | Yes |
| Participation in threat-led penetration testing | 30(3)(d) | Critical or important | Gap | Yes | Gap |
| Ongoing monitoring with unrestricted access, inspection and audit rights | 30(3)(e) | Critical or important | Yes (pooled) | Yes | Yes |
| Exit strategy with a mandatory adequate transition period | 30(3)(f) | Critical or important | Yes | Yes | Yes |
Legend: Yes means the element is present; Gap means a mandatory
element is missing. The 30(3) elements apply to every arrangement for a
service supporting a critical or important function, so CA-0005 (F3)
needs the TLPT cooperation clause even if the entity is not yet
identified for TLPT under Article 26(8).
Gaps: [list each gap with the remediation route, for example contract
amendment at renewal, side letter or compensating control, and the
target date]. Delegated Regulation (EU) 2024/1773 sets the content of
the policy that governs these arrangements, including governance, the
ex-ante risk assessment, due diligence, conflicts of interest,
contractual clauses, monitoring, and exit and termination.
12. Definitions template
| Template and column | Value | Entity definition |
|---|
| B_07.01, impact of discontinuing the ICT services (Example) | High | Interruption beyond the function RTO would stop client payments or breach a regulatory deadline |
| B_07.01, impact of discontinuing the ICT services (Example) | Medium | Interruption beyond RTO degrades service but a manual workaround exists for at least five working days |
| B_07.01, impact of discontinuing the ICT services (Example) | Low | Interruption has no client impact within ten working days |
| B_02.02, sensitiveness of the data stored (Example) | High | Client personal data, payment data or confidential supervisory information |
13. Data quality controls
| Principle | Check | Method | Frequency | Owner |
|---|
| Accuracy | LEIs valid and active; EUIDs well formed | GLEIF lookup; format check | Each update and pre-submission | Procurement |
| Completeness | Every ICT services contract in the contract system has a register entry; all mandatory columns filled | Reconciliation report | Monthly | Third-party risk lead |
| Consistency | Every reference in B_02.02, B_03, B_04, B_05.02 and B_07.01 exists in B_02.01; every provider code exists in B_05.01; every function ID exists in B_06.01 | Referential checks | Each update | Register owner |
| Integrity | Entity-level registers agree with the consolidated register | Group reconciliation | Quarterly | Group reporting |
| Uniformity | Closed-list values match the current list of possible values; dates and currencies use the required formats | Validation rules of the reporting framework | Pre-submission | Group reporting |
| Validity | Rank 1 only for direct providers; end date after start date; critical function rows have a B_07.01 entry | Rule set | Each update | Register owner |
13.1 Pre-submission checklist
- Run the competent authority's or EBA validation rules on the export and
clear every blocking error.
- Confirm that the reference date and the date of reporting are correct.
- Confirm that every function marked critical or important has at least
one arrangement and an assessment.
- Obtain sign-off from the owner and the accountable management body
member.
14. Reporting, notifications and critical providers
14.1 Submission record
| Collection | Reference date | Deadline to competent authority | Format | Submitted | Validation result |
|---|
| [Year] (Example) | YYYY-MM-DD | YYYY-MM-DD (per authority instructions) | [Plain CSV reporting package per the EBA reporting framework / authority portal] | YYYY-MM-DD | [Accepted / Rejected, errors] |
For the first collection the ESAs asked competent authorities to report
the registers to them by 30 April 2025 (JC 2024 99). In 2026 the
reference date was 31 December 2025 and authorities collected registers
between February and March 2026 on national deadlines. Confirm the
reference date, window and validation rules in your authority's notice
each year.
14.2 Notifications to the competent authority
| Event | Requirement (paraphrase) | Record |
|---|
| Planned arrangement for a service supporting a critical or important function | Inform the authority in a timely manner (Article 28(3)) | [Date, reference] |
| A function becomes critical or important | Inform the authority (Article 28(3)) | [Date, reference] |
| Request for the full register or sections | Provide on request (Article 28(3)) | [Date, scope] |
14.3 Critical ICT third-party service providers
The ESAs designate critical ICT third-party service providers (CTPPs)
under DORA Article 31 and publish the list. The first list, published in
November 2025, named 19 providers. Mark each provider in the register
that is on the current list, because a designated provider is overseen
by a Lead Overseer and the entity may receive information or
recommendations that affect its arrangement.
| Provider | On CTPP list | List date checked | Effect on arrangement |
|---|
| PRV-002 (Example) | [Yes / No] | YYYY-MM-DD | [Lead Overseer recommendations reviewed / None] |
Check the current list on the ESAs' DORA oversight pages; it is updated
over time.
