| Near-miss reporting culture | Near misses reported per 1,000 hours worked | [Set after month 1] | Monthly review |
| [Aim] | [Measure] | [Target] | [Forum] |
4. Management of the work
4.1 Management structure and responsibilities
- The site manager is responsible day to day for implementing this
plan, briefing site rules and stopping unsafe work.
- Each contractor supervisor briefs their RAMS to their own team before
work starts and records the briefing.
- The temporary works coordinator keeps the temporary works register,
checks designs and issues permits to load or strike.
- Any person on site may stop work they believe is unsafe without penalty.
4.2 Cooperation and coordination
| Forum | Frequency | Chair | Attendees | Output |
|---|
| Daily coordination briefing | Daily, before work | Site manager | Contractor supervisors | Agreed work areas, permits, interfaces; recorded in the daily site report (construction/daily-site-report) |
| Weekly contractors' meeting | Weekly | Project manager | All contractors, principal designer as needed | Two-week look-ahead, RAMS status, inspection actions |
| Design and risk review | At each design change | Principal designer | Designers, principal contractor | Updated risk information to section 8 |
4.3 Permits to work
| Permit type | Issued by | Applies to |
|---|
| Hot work | Site manager | Welding, cutting, grinding, torch-on roofing |
| Excavation and breaking ground | Site manager after service location | Any dig deeper than [300 mm] |

| Lifting operation | Appointed person (lifting) | All crane lifts; each lift follows a lift plan |
| Electrical isolation | Authorised person | Work on or near live systems |
4.4 Design changes and site information
Design changes reach site through the request for information process
(construction/rfi-log) and the change process
(construction/change-order-variation). Before a changed detail is built,
the site manager checks whether it changes a hazard in section 8 and, if
it does, updates the relevant RAMS and this plan.
4.5 Site security and public protection
[Describe the hoarding line, gates, out-of-hours security, and protection
of the public and neighbours.]
Example: 2.4 m solid hoarding on the public boundary; two gated
entrances, each with a banksman during deliveries; deliveries scheduled
outside school arrival and departure times (08:15 to 09:00 and 15:00 to
15:45); CCTV and intruder alarm out of hours.
5. Site rules
| ID | Rule |
|---|
| SR-01 | Sign in on arrival and sign out when you leave, every day |
| SR-02 | No work before site induction; visitors are escorted at all times |
| SR-03 | Minimum PPE everywhere beyond the welfare area: safety helmet, high-visibility vest, safety boots, gloves and eye protection; task-specific PPE as stated in your RAMS |
| SR-04 | Do not start an activity until its RAMS has been accepted and briefed to you |
| SR-05 | Use designated pedestrian routes; keep out of the exclusion zones of plant and lifting operations |
| SR-06 | Do not alter, remove or overload any scaffold, edge protection or temporary works; report damage |
| SR-07 | Hot work only under a hot work permit, with a fire watch for [60] minutes after work stops |
| SR-08 | No smoking or vaping except in the designated area |
| SR-09 | Mobile phones are not used while operating plant or working at height |
| SR-10 | No alcohol or drugs; prescribed medicine that can affect safety is reported to your supervisor |
| SR-11 | Report every accident, near miss and unsafe condition the same day |
| SR-12 | Keep work areas tidy; clear waste to the designated skips at the end of each shift |
| SR-13 | Parking only in the contractor car park; no parking on the school access road |
| [SR-nn] | [Rule] |
6. Induction, training and competence
6.1 Site induction content
